LAUSD FOOD TRUCK REQUIREMENTS

- Certificate of Insurance
- Health Permit
- Business License
All Certificates of Liability Insurance must list LAUSD as the certificate holder and as an additional insured, SEE BELOW.
The Certificate Holder portion of the insurance certificate must be listed as follows:
Los Angeles Unified School District & the Board of Education of the City of Los Angeles 333 South Beaudry Ave, 28th Floor Los Angeles, CA 90017 AND Additional Insured Endorsement must be noted as follows: The Commercial General Liability policy and the Commercial (Business) Automobile policy must contain an additional insured endorsement in favor of: “Los Angeles Unified School District and the Board of Education of the City of Los Angeles”
This of you have worked with LAUSD know they will not allow you on the property unless these are provided. Please get these to me sooner than later in the event LAUSD has an issue with anything (and they typically do).
These documents are required to be submitted back at least 7 days prior to the booking. If you do not comply, you lose your booking and your fee. No exceptions.
LAUSD VENDOR REQUIREMENTS

Pop-Up Vendors are required to submit a Waiver within 7 days of their booking.
